Findings and repair sequence
The boiler pressure relief valve is leaking and requires replacement. The boiler is currently not firing. Replacement of the leaking relief valve is the first repair required to safely refill and pressurize the system, then test boiler ignition, safety controls, circulation, expansion tank, and automatic makeup-water system. Additional repairs are not included and require separate customer approval.

Terms
A 50% down payment of $437.50 is due upon signing this authorization and before scheduling the repair or ordering materials. The deposit is credited toward the total authorized price of $875.00. The remaining $437.50 is due upon completion of the authorized services. Payment instructions are provided separately. This signing page does not process payments. Any applicable statutory cancellation or deposit-refund rights take precedence. The valve will be matched to the boiler's specifications. Maintenance includes appropriate inspection, cleaning, and purging. Additional repairs require separate approval. A 20% restocking fee on specially ordered materials applies only where legally permitted and does not override statutory cancellation rights.
Where this transaction is entered into while the contractor is physically present in the customer's residence, applicable Illinois in-home cancellation rights and required notices apply. For qualifying in-home transactions, the three-business-day cancellation notice and two required copies should be provided separately.
